FollowMyHealth Sign Up
If you have had an appointment with one of our providers recently, you
will receive an automatic invite sent via e-mail if:
- Your GWMFA patient account has a valid email address on file
- You are not currently a FMH member
- You have not rejected an FMH invitation in the past
- Patients are unable to sign up for FMH on their own. Automatic invites go out each night.
If you do not find the invitation email in your Inbox, please check your
Spam or Junk folder.
- NOTE: Some email providers, such as those associated with work/government, employ strict filtering methods. Update your email address with our Front Desk staff when you visit any of our clinic locations.
- You will be required to enter an INVITATION CODE, which will be given to you in your email. Please note: the invitation code is only active for 30 days.
